The St. Jamestown Outreach Program (STOP) is an Aging-at-Home initiative – funded through the TC-LHIN Aging at Home Strategy that provides services to older adults (55 and older) who live in the Toronto Community Housing apartments in St. James Town, and who are experiencing significant challenges in their mental health, housing stability, addictions and/or physical health.
Presenting issues include social isolation, deterioration of physical health, and risk of eviction, hoarding and infestation.

Anyone may make a referral, including the person in need of service. The person being referred must meet the criteria and be in need of intensive long-short term community support and live in St. Jamestown.
